Level-Funded Options + Add level-funded
A self-funded plan that bills like a premium. Employees pay a contribution and have plan cost-sharing exactly like a group plan (the contribution method on the left applies). Claims % = share of the rate that funds claims (the at-risk/refundable portion; rest is stop-loss + admin); Surplus % = portion of an unused claims fund refunded in a low-claims year; MaxAgg % = aggregate stop-loss attachment (e.g. 125% caps a bad year). The comparison shows the expected employer cost with a low-claims / high-claims band.
Individual-Coverage HRAs + Add ICHRA+ Add QSEHRA
Type sets ICHRA (no cap, any-size employer, may vary by class) or QSEHRA (<50 FTEs, no group plan, statutory cap). IP = the individual-market monthly premium for that tier; Allow = the employer’s monthly allowance. Employer cost = allowance, capped at the premium (and, for QSEHRA, at the statutory cap); employee pays premium − allowance.
Cash-in-Lieu (Taxable) + Add cash-in-lieu
A taxable-cash baseline: the employer pays cash instead of a benefit and the employee buys their own coverage. IP = the full individual-market premium the employee would pay; Cash = the gross monthly cash. Employer cost grosses the cash up for payroll tax (rates in the sidebar); the employee’s net cost is the premium minus the after-tax cash. There’s no payroll-tax break — so this exposes the tax cost of cash versus a pre-tax option. The cash must be unconditional wages; tying it to insurance is a prohibited Employer Payment Plan ($100/day/employee excise).
